Therapy & Early Intervention Referrals
Pediatric Therapy Referrals
Physicians:
Physicians can fax referrals to PLC at (901) 443-1718.
Once we receive the referral, our office will email the family a link to complete their intake paperwork.
Parents & Caregivers:
You can call our office at (901) 290-8558 to get started.
We'll contact your child's pediatrician to request a prescription for evaluation and treatment.
We'll also verify your insurance eligibility and coverage. Once we have your completed intake paperwork, prescription, and insurance approval, your child's therapist will contact you to schedule the initial session.
Early Intervention (TEIS) Referrals
Anyone can refer a child to the Tennessee Early Intervention System (TEIS), including parents, physicians, and childcare providers.
Once a referral is submitted, the TEIS referral team will process it, and an Eligibility Evaluation Agency will contact the family to schedule an evaluation.
TEIS will also request medical records from the child's physician.
If your child qualifies for services, you'll be assigned a TEIS Service Coordinator who will work with your family to develop an Individualized Family Support Plan (IFSP) based on your child's needs.
